Forms and Documents

Everything you need in one spot. Please read instructions carefully and complete required paperwork early so we are not chasing it last minute.

Start here: complete the required forms first, then use the packing and preparation documents to get ready.

Trek Forms Checklist

Complete all required forms by April 12, 2026.

  2. Church Permission Slip (must be printed, signed, and turned in as a physical copy)

Registration form additions

Youth code of conduct: I agree that I will exemplify Christlike behavior while on Trek and have read and agree to follow the standards found in “For the Strength of Youth” as it is currently published in the LDS Gospel App.

Parent code of conduct: I agree that I have reviewed these standards with my youth and my youth can be held to the standards and behaviors indicated in “For the Strength of Youth” published in the LDS Gospel App.

Photo release question (yes/no): I grant permission and consent for use of all Trek photos, video and media to be shared within church media and contacts.

Required Form Links and Turn-In

Use these links and instructions to finish your paperwork. All items are due by April 12, 2026.

  • Required – do first

    Registration Form

    Complete online. Includes code of conduct agreement and a photo/media release yes-or-no question.

  • Required – print and sign

    Church Permission Slip

    This MUST BE printed, signed, and turned in as a physical copy to local unit leaders. It can also be turned in to stake youth leaders at Stake Conference on April 12.

  • Sports Physical

    Turn in a physical copy to local leaders, or email a photo/scan to Jessica Crane at jessloves2run@gmail.com.

  • Parent Letter to Your Youth

    Please write a letter your youth will open during Trek. Consider sharing your testimony and all the great things you love about them. The letter will only be read by your youth. Place it in an envelope with their name and hand it to local unit leaders, or email it to jessloves2run@gmail.com to be printed. You may also mail letters to Jessica Crane, 2106 Bradford Drive, Montrose, CO 81401.

Packing and Preparation

Use these documents to plan clothing, gear, training, and spiritual preparation. Keep it simple and practical.

Lost Property

The stake accepts no responsibility for personal items that are lost or stolen during the Trek.

Do Not Bring

Cell Phone, Candy or any other food items, radios, pillows, sleeping pads, electronic items, shampoo (biodegradable soap will be provided), any type of jewelry, or anything that does not appear in the above equipment list.

Pioneer Story Preparation

Please research and bring along a pioneer name with you. This can be a family member or any inspiring pioneer and their story. Please take time before Trek to learn details about their life.

FAQ

Where do I submit completed forms?

Registration is submitted online. Church permission slips and sports physicals can be turned in as physical copies to local leaders, and permission slips can also be turned in to stake youth leaders at Stake Conference on April 12. Sports physical scans can be emailed to jessloves2run@gmail.com. Parent letters may be handed to local leaders, emailed to Jessica, or mailed to Jessica Crane at 2106 Bradford Drive, Montrose, CO 81401.

What if I have medical concerns or special needs?

Submit information as early as possible and contact Medical lead Justin Tanner at justintannermd@gmail.com or (303) 906-4201. Early communication helps us plan medications, allergies, and accommodations safely.
